• Form I-9 Employment Eligibility Verification

•  One of the following:

–   AO 196A - Acknowledgment of Gratuitous Services and Waiver (court volunteer that is not a student)
–   AO 196B - Acknowledgment of Gratuitous Services and Waiver (chambers or court volunteer that is student)
–   AO 196C - Acknowledgment of Gratuitous Services and Waiver (chambers volunteer that is not a student)
–   AO 196D - Acknowledgment of Gratuitous Services and Waiver (former employee serving as a volunteer)
